WAIVER, RELEASE OF LIABILITY, AND PHOTO RELEASE FORM

AAMF x AZ Art Studio

1. Acknowledgment of Product Characteristics

Handcrafted Items : I understand that handmade products may have natural imperfections, such as minor variations in color, texture, or shape, which reflect the artistry and craftsmanship.

Makeup Products : I acknowledge that makeup may contain ingredients that could cause allergic reactions or skin irritation, depending on individual sensitivities.

Clothing : Fabrics, dyes, or embellishments may trigger allergic reactions or discomfort for certain individuals.

Herbal Products : I understand that herbs may interact with medications or cause allergic reactions, and I accept the responsibility of consulting a healthcare professional before use.

2. Assumption of Risk

I voluntarily assume all risks related to the use of these products, including:

- Allergic reactions, irritations, or discomfort caused by materials or ingredients.

- Variations or imperfections in handcrafted items.

- Reactions due to improper use or failure to follow instructions.

3. Waiver of Liability

I release and discharge AAMF x AZ Art Studio, its owners, employees, and partners from any liability, claims, or damages arising from the use or purchase of these products. This includes all foreseeable and unforeseeable risks.

4. Photo Release

I grant AAMF x AZ Art Studio permission to take and use photographs or videos of me and/or my purchased products for promotional, marketing, or educational purposes, in both print and digital media, including but not limited to social media platforms, websites, and advertisements.

- I understand that I will not receive compensation for the use of these images.

- If I do not wish to be photographed, I will notify **AAMF x AZ Art Studio** in advance.

5. Personal Responsibility

- I will carefully read product descriptions, including materials, ingredients, and care instructions.

- I will discontinue use and seek medical attention if I experience any adverse reaction.

- I am responsible for the proper care and storage of the items I purchase.

6. Hanfu Rental and Deposit Policy

I, the undersigned, acknowledge that I have inspected the Hanfu clothing and associated hairstyle accessories. I accept them in their current condition. I understand that I am responsible for any stains, damage, or excessive cleaning required due to my use of the clothing and accessories. I agree that any costs associated with repairs or cleaning will be charged to me. Additionally, I recognize that a deposit may be required and will be refunded if no damage occurs. By signing below, I release the rental provider from any liability related to the clothing and accessories.

7. Agreement and Signature

I have read, understood, and voluntarily agree to the terms outlined in this waiver, including the product liability and photo release sections. This waiver applies to all purchases from AAMF x AZ Art Studio and is legally binding.
